Our critical illness benefits help relieve financial strain when someone is diagnosed with a covered critical illness as enumerated and defined in the policy by providing a lump sum payment, to be used however the insured person* chooses.
With this policy, critical illnesses are grouped into categories. Each Insured person has a maximum benefit amount per category as shown in the policy schedule. In the event an insured person is diagnosed with a covered critical illness, we will pay the benefit provided under the policy.
Worksite Critical Illness is a limited benefit policy. It is not intended to replace major medical, medical expense or Medicare Supplement coverage. The Critical Illness policy does not meet the Minimum Essential Coverage Requirements of the Affordable Care Act.

Elected at Employer Level
Cancer Critical Illness Benefit Rider – This rider adds coverage for certain types of cancers as enumerated and defined in the rider.
Elected at Employee Level
Wellness Benefit Rider2 – The selected benefit amount will help pay for the cost of one health screening test selected from a list contained in the policy. The benefit amount will be paid only once per calendar year and for only one covered person per policy. Covered screening tests may vary by state.
Recurrence Benefit Rider – This rider pays a benefit if the insured person is diagnosed with a recurrence of the same covered critical illness after being Symptom and Treatment Free as defined in the rider for 12 months. Please note, the Recurrence Benefit Rider excludes dependent child critical illnesses and certain other critical illnesses as enumerated in the rider. A recurrent critical illness benefit is payable only once per category for each insured person.
Cancer One Recurrence Benefit Rider – If the employer decides to offer the Cancer Critical Illness Rider, and if the employee selects the Recurrence Benefit Rider, the Cancer One Recurrence Benefit Rider also will be attached to the policy. This rider pays a benefit of the insured person is diagnosed with certain types of cancer a second time after being Symptom and Treatment Free as defined in the rider for 12 months. Please note, the Cancer One Recurrence Benefit Rider excludes certain cancers covered by the Cancer Critical Illness Rider.
